Gwybodaeth a Chyngor i Rieni / Information and Advice for Parents

Mae'n bwysig iawn ein bod ni i gyd yn gweithio gyda'n gilydd mewn partneriaeth er mwyn sicrhau'r canlyniad gorau ar gyfer eich plentyn.

 

Mae hyn yn galw am ymddiriedaeth, ymrwymiad a pharch, ac mae meithrin cydberthynas gadarnhaol rhwng pob unigolyn sy'n gweithio gyda'ch plentyn yn fater pwysig iawn.

 

Mae'n hanfodol fod pob partner yn chwarae rhan yn y gwaith o feithrin cydberthynas gadarnhaol ac adeiladol gyda'r ysgolion yn ogystal â'r Awdurdod Lleol, asiantaethau allanol a rhieni.

 

Mae'r gylchred barhaus yma yn cynnwys pedwar cam pwysig mewn perthynas â diwallu anghenion dysgu ychwanegol plant. Mae'r camau yma yn cynnwys:

  • Nodi ac asesu anghenion eich plentyn
  • Cynllunio darpariaeth addas
  • Rhoi camau gweithredu cytûn ar waith sydd â'r nod o sicrhau deilliannau amlwg
  • Adolygu'r ddarpariaeth er mwyn sicrhau ei bod hi'n diwallu anghenion eich plentyn

It is really important that we all work together in partnership to make sure we get the best outcomes for your child.

 

To do this, it requires trust, commitment and respect, with particular importance on building positive relationships between everyone involved with your child.

 

It is essential that all partners involved play a role in developing a positive and constructive rapport not just with the schools but with the Local Authority and outside agencies as well as with parents.

 

The continuous cycle of this process has four very important stages in meeting children’s additional learning needs. These stages include:

  • Identification and assessment of your child’s needs
  • Planning suitable provision
  • Implementing agreed actions with clear intended outcomes
  • Reviewing the provision to ensure it is meeting your child’s needs
